Congestive Heart Failure (CHF) and Heart Disease
Congestive heart failure happens when the heart's weak pumping action causes a buildup of fluid called congestion in your lungs and other body tissues. Heart failure does not mean that your heart has completely stopped pumping blood; it means that your heart is not pumping all the blood that your body needs to function normally.
If you recently have been diagnosed with CHF, or you are visiting the hospital or ER more often than you would like, consider homecare from Residential. Under the care of Residential Home Health, a dedicated care team of nurses and therapists will help you remain stable at home, with a treatment plan including:
- Provide a full physical assessment during each visit to monitor for complications
- Measure patient’s weight and size of legs, ankles, feet and abdomen to assess water retention
- Teach proper use of medications
- Assist and educate patients in safely performing activities of daily living
- Teach energy conservation skills to help patients maintain higher energy levels
- Assess the safety of patients’ homes, then recommend and help implement safety improvements
Many of Residential’s CHF patients are eligible for eCareRN, our in-home telehealth system for daily monitoring of vital signs and health status.
